Access to your records is provided through our secure Leap patient portal. Our office must send you an invitation email with a link and activation code. Once you receive it, you can register and view your records. If you have not received the invitation, please let us know and we will resend it—patients cannot register without the activation code.
Either your provider can fax those to us, or you can upload your records via portal, fax or secure email.

With HMO plans, the patient’s Primary Care Physician (PCP) is the only provider who can issue the referral, and the patient is ultimately responsible for making sure the referral is in place before the visit. Our office will help by communicating with your PCP and providing the information they need, but some HMO plans require you to schedule a PCP appointment first before they will approve a referral. To avoid delays in your care, please contact your PCP as soon as possible and confirm that the referral has been submitted to our office.
You can log in to the portal and see your appointment day and time and can send us a message if you need to change that via your portal.
This is something that will need to be verified by our administrative team, if your insurance card stated an amount for a specialty visit then that helps and if not we will need to get that from your insurance by either electronic request or by calling, but it will help if you call your insurance directly and ask for your portion cost, the insurances respond better to the customer than providers.
